Streamline Empire Ltd (Ms Curvaceous UK) Customer Product & Service Agreement
Streamline Empire Ltd supplies all products and services promoted and sold through the brand Ms Curvaceous UK. Streamline Empire also communicates to customers using the Ms Curvaceous UK Brand name.
I understand information supplied on this form gives Streamline Empire Ltd permission to communicate with me by mail, social media, email, or phone.
Please read the following carefully from beginning to end. In order to qualify for a refund or a date change, you must follow the important policies and procedures outlined below:
I further understand that the product or service I’ve purchased contains confidential copyrighted information that may not be copied or reproduced without written consent from Streamline Empire Ltd.
We offer educational, inspirational and entertaining content; newsletter and promotional emails as a service to our customers. We will send you text messages and emails periodically outlining special events, current products, specials and deals. However, you may ’opt-out’, or unsubscribe from our text messages or emails by following any unsubscribe instructions in any email you receive from us. After doing so, if you have opted out of receiving all text messages and promotional emails you will not receive future promotional emails, or text messages (as applicable), unless you open a new account, attend an event, enter a contest, or otherwise subsequently sign up to receive newsletters, our curated content or emails.
Request for Refund
I understand all sales over £100 in value are considered final 5 business days from date of product or service receipt. This includes any receipts I receive when a payment plan for a product or service has been set up. The 5 business day time frame still applies if I have not yet made the first payment of said payment plan. Should I request a refund or cancellation on or before 5 business days from the date of sale, I will supply written notice of said request to info@mscurvaceous.com at Streamline Empire Ltd. I will also call Streamline Empire Ltd on 02033225888 and request return instructions. All cancellations and refunds must be authorised by Streamline Empire Ltd.
I understand that all sales under £100 are non-refundable unless cancellation, or date change of the event occurs at the responsibility of Streamline Empire Ltd.
Extenuating circumstances policy and the coronavirus (COVID-19) - Please be aware that we do not offer any refunds on admin fees if the event is cancelled. We are able to offer a date change free of charge instead. This is a strict policy with no exceptions.
Date Changes
I understand that date changes are only considered before the set date the product or service is set to be supplied by Streamline Empire Ltd. Should I request a date change before the set date the product or service is being supplied, I will supply written notice of said request to info@mscurvaceous.com with my reasons for the date change.
I understand that if I wish to change the date set for me to receive a product or service supplied by Streamline Empire Ltd I may have to pay an admin fee. I understand that the admin fee will be 33% of the standard cost of the product or service or £30 depending on which of the two is lower.
I understand the admin fee is non-refundable and I waiver my right to a refund should Streamline Empire Ltd cause there to be another date change or cancellation of the date set for the product or service I have been transferred onto.
Further Product Return Requirements and Conditions:
I understand ALL products and Services, including Bonus items, must be returned to Streamline Empire Ltd in ORIGINAL condition. Streamline Empire Ltd staff, officers, volunteers or agents will not be held responsible for any damage incurred during shipment of returned products to Streamline Empire Ltd.
I further understand:
1. Written detailed reasons for cancellation should accompany my written notice of cancellation.
2. A photocopy of, or digital email version of my sales receipt indicating payment type must accompany my written notice of cancellation.
3. My name, address, telephone number and email address must be legible and contained within my written notice of cancellation.
4. Name of the shipping company used and any tracking information (tracking number) must be legible and contained within my written notice of cancellation.
Approval of Refund Request
I understand there will be no consideration or authorisation for a refund, cancellation or date change without meeting all requirements and conditions outlined above.
I understand upon approval of my refund request and receipt of returned product or service by Streamline Empire Ltd, payments made under this written contract of sale will be refunded within 14 business days.
I understand purchased product or service cannot be returned at subsequent seminars, symposiums or training meetings of Streamline Empire Ltd or other PRODUCT OR SERVICE PROVIDERS. Streamline Empire Ltd will not be responsible for honouring refunds that do not follow the process outlined above.
